There are four types of applications you can fill out to become registered or change your registration status.
Practitioners must be registered in one of three registers: General, Provisional and Courtesy.
Practitioners must fulfill the registration requirements to hold an active practice permit.
Reinstatement is for former members who want to reinstate their practice permits.

It is the professional obligation of regulated members to self-report any breaches in their own conduct.
To report a complaint, access the Public/Patient and/or Employer’s Complaint forms.
